The Many Benefits of Hormone Replacement Therapy (BHRT)

The benefits of bioidentical hormones far outweigh the risks. Bioidentical hormones are most commonly prescribed to treat symptoms of hormonal imbalance in women and men, such as hot flashes, night sweats, loss of libido, fatigue, weight gain, mood swings, and irritability. Many doctors believe that women and men who opt for bioidentical hormone replacement therapy (BHRT) instead of undergoing conventional (synthetic) hormone replacement therapy (HRT) will experience fewer adverse reactions. The side effects of BHRT are usually mild if present at all.

Some of the benefits of using bioidentical hormones include:

  • Reduced risk of osteoporosis and improved bone density
  • Reduced incidence of hot flashes and vaginal dryness
  • Better maintenance of muscle mass and strength
  • Improved cholesterol levels
  • Reduced risk of endometrial and breast cancer
  • Reduced risk of depression
  • Improved sleep
  • Better mood, concentration and memory
  • Improved libido
  • Reduced risk of Alzheimer’s disease

Many patients opt for BHRT because it is not a one-size-fits-all approach to healthcare. Each bioidentical hormone prescription is custom-compounded, based on each patient’s individual diagnostic results, making bioidentical hormones safer and more effective in comparison to conventional (synthetic) hormones.

BHRT TREATMENT IS FOR MEN & WOMEN:

  • Safer
  • Less risk of breast cancer
  • Less liver metabolism of hormone so less cancer-causing by-products
  • Equally effective as traditional hormone replacement therapy
  • By prescription only
  • Customized dosage to your needs
  • Available in many forms including implantable hormone pellets
  • Uses multiple hormones for restoration of natural hormonal balance

A common misconception about BHRT is that it’s all the same and it all causes the same risks. The media often confuses similar terms in their reports of negative outcomes and needlessly steers patients away from this important therapy.

In 2012 the North American Menopause Society (NAMS) released their HRT position statement supporting HRT for symptomatic female patients and even endorsed bio-identical hormones as having distinct safety and dosing advantages. Conjugated equine estrogen (CEE) and Medroxyprovera (MPA) are two non-identical hormones that were studied in the large Women’s Health Initiative (WHI) and were found to have significant breast cancer rates when administered together.

We later found out that this combination produces cancer causing estrogen metabolites that turn on cancer genes in the breast. The media made this very public but never mentioned that even CEE given alone actually had a reduced risk of breast cancer. Further large studies on bio-identical estradiol andВ bio-identical progesterone showed null risks of breast cancer. This difference in the medications used if fundamentally important to BHRT:

  • Estradiol is the hormone that is exactly identical to that made by the body
  • Estrogen include CEE which are completely different in structure and thus have a different risk profile
  • Progesterone is the bio-identical version of the progestin group which also includes MPA, the non-identical hormone now linked to increasing breast cancer as part of HRT protocols using this medication.

Other bio-identical hormones used include testosterone and DHEA, which are the androgens and are present in women in small amounts. Testosterone can also be metabolized to estrogen in the female body and is sometimes used for that purpose. The androgens are generally very safe in women.

BIOIDENTICAL ESTROGEN

At Vitamore, Dr. Paukman utilizes a prescription formula of several bio-identical hormones that are prepared into implantable pellets and placed under skin.

Treatment of Low Testosterone

Natural Testosterone replacement therapy is quickly becoming very popular among aging men. At Vitamore BHRT, Dr. Paukman takes a unique and highly effective approach to hormone replacement using implantable testosterone pellets. TESTOSTERONE REPLACEMENT THERAPY IN THE AGING MALE HAS BEEN SHOWN IN CLINICAL TRIALS TO IMPROVE THE FOLLOWING:

  • Increase lean body mass
  • Decrease fat
  • Increase strength
  • Increased libido

At Vitamore, Dr Paukman uses small implantable testosterone pellets that are placed under the skin and provide a consistent, reliable, continuous dose of testosterone. This is very natural to your body since your body is constantly exposed to testosterone. These prescription pellets are FDA approved and even recommended as a treatment option by the guidelines of multiple medical organizations.

Pellets Therapy

Benefits ofВ Testosterone Pellets:

  • Safe and approved medication
  • Continuous testosterone delivery
  • Customizable dosing
  • Highly effective
  • Blood levels rise within hours of implantation
  • NO fluctuations in levels of testosterone
  • NO daily dosing
  • NO creams
  • NO chance of transferring testosterone to close contacts

Testosterone has even been used to treat male bone loss, anemia, and some studies have shown improved outcomes in heart disease patients undergoing testosterone replacement therapy.

Treatment Side Effects

Testosterone replacement is very safe when done under the guidance of a trained physician who will closely monitor blood levels and other parameters. These occur rarely and are usually mild and can be managed with simple interventions or dose adjustments.

Side effects may include:

  • Swelling
  • Breast tenderness
  • Mood changes
  • Acne
  • Headache
  • Enlargement of prostate

Serious Side Effects may include:

  • Elevated blood pressure
  • Elevated cholesterol
  • Elevated red blood cells

Testosterone therapy DOES NOT cause prostate cancer. Researchers at Harvard have shown that it takes a very small amount to testosterone in the body to cause prostate cancer and beyond that level the amount testosterone does not matter. That level is roughly 100 (very very low) so a man with Low T at a level of 250 is already at the same risk for prostate cancer as a man with a level of 900.

These side effects require close monitoring and may require more serious interventions.

Contraindications to Bioidentical Testosterone Therapy Include

  • Current or previous history of prostate cancer
  • Certain types of blood disorders
  • Severe benign prostatic hypertrophy

MENOPAUSE AND HORMONE REPLACEMENT THERAPY

Bio-Identical Hormone Replacement Therapy (BHRT) is a highly effective treatment for menopause and its many symptoms. It is even used by post-menopausal women for longevity and improvement of quality of life symptoms that lack of hormones causes. BHRT is uniquely different from other traditional forms of hormone replacement because the bio-identical hormones are identical to the hormones your body made. This “form-fits-function” approach to HRT provides many benefits over traditional hormone pills or injections. Menopause is a natural part of aging in women and every woman will reach menopause in her lifetime.

THE SYMPTOMS OF MENOPAUSE AND ITS SEVERITY WILL BE DIFFERENT IN EVERY WOMAN BUT CERTAIN COMMON FEATURES ARE:

  • Hot flashes
  • Moodiness and crying spells
  • Difficulty sleeping (insomnia)
  • Anxiety
  • Palpitations (rapid heart beat)
  • Vaginal Dryness
  • Loss of interest in sex
  • Loss of ability to achieve orgasm
  • Pain with intercourse
  • Bleeding after intercourse
  • Thinning of skin, decreased elasticity of skin, increased wrinkles
  • Burning with urination
  • Urinary incontinence (such as with laughing or coughing)
  • Weight gain
  • Memory problems and difficulty concentrating

While these are certainly very bothersome symptoms, most of them will not negatively impact your overall health.

MENOPAUSE DOES HOWEVER, CARRY WITH IT SEVERAL VERY SERIOUS WOMEN’S HEALTH ISSUES:

Increased heart disease Bone loss and fractures (osteoporosis) Age for reaching menopause ranges from 40 to 58 with 51 being an average age a woman enters menopause. As a woman ages, her ovaries lose their ability to produce estrogen and progesterone and the brain tries to increase that production by producing stimulatory hormones: LH and FSH. Rising levels of these hormones are a hallmark of menopause and aid in diagnosis. The levels of LH and FSH keep rising as the ovaries do not respond with sufficient amounts of estrogen and progesterone. This process is gradual and occurs over months to years.

Hormone levels often fluctuate and periods become irregular and then cease as the levels become too low. The fluctuations in hormones often give rise to symptoms which typically worsen as levels decrease. Most cells in the body have estrogen and progesterone receptors and thus not having those hormones leads to a wide array of symptoms. For example, hot flashes occur when estrogens tops regulating the body’s thermostat center in the brain, libido decreases as other hormones drop along with estrogen and progesterone, vaginal mucosa atrophies as estrogen ceases to maintain proper tissue environment, etc.
